Vaccination Documentation Upload

 

GENERAL INFORMATION

Sundance Saloon requires documentation of full vaccination to attend. You must have had your qualifying vaccine dose 14 or more days prior to the event date. You will also need to bring a government-issued photo ID to Sundance Saloon. (Do not upload your ID!)

If you attend Sundance Saloon regularly, we highly recommend that you upload your vaccination documentation in advance for faster entry. Once we have received your documentation, you will be set for future events with Sundance, including the Sundance Stompede, as long as our vaccine requirements remain unchanged.

IMPORTANT: Please allow 24 hours for processing. If you send us your documentation on the same day as the event you wish to attend, we won't have time to review and process your submission. In any case we suggest you still bring your proof of vaccination with you as a back-up.

We will require at least one of the following:

  • PREFERRED: A screenshot image of your California Digital Covid-19 Vaccine Record including the QR code. - OR -
  • A clear photo or scan of the front and back of your CDC vaccination card. Your full name, vaccine type, and all inoculation dates must appear. - OR -
  • An image of any government-issued vaccination record.

If the name on your vaccination card differs from the name on your photo ID, please let us know that in advance (info@sundancesaloon.org).

We will remove all files from our secure location in the cloud and store them locally. Your name and email address will be used for contact tracing purposes only, or to contact you if we have a question about your upload.

Recommended for regular visitors to Sundance Saloon.
